Trauma-informed, every mentor
Every mentor has trauma-informed training. Not a workshop. A practice discipline.
Supporting Communities Australia is a Western Sydney NDIS provider serving young people aged 12 to 20 with disabilities and trauma backgrounds. We work in homes, schools, and the community, one young person at a time.
SCA was founded in 2025 by Yosmerys Aparicio Cuencas, an immigrant woman from Venezuela with a Bachelor's in Criminology and a Diploma of Counselling from 4Life College. Yos built SCA because she saw the same pattern in too many services: well-meaning providers who worked on young people instead of with them.
SCA is the operational arm of the Elevate Youth Lab model, a practice framework built on five principles: rapport before skill, capacity before compliance, explicit teaching, supports-included independence, and neuroaffirming practice.
Most NDIS providers focus on hours delivered. We focus on capacity built. That is a harder metric to measure, but it is the only one that matters when a young person is sitting in front of you.
Every mentor has trauma-informed training. Not a workshop. A practice discipline.
We work with parents, carers, coordinators, and DCJ as one team. Weekly updates come standard.
We do not pretend behaviour is separate from environment. Sessions happen where the young person actually lives.
We share our thinking publicly. Our blog and NDIS guide show how we approach the work.
Read our blog and NDIS guide for how we approach the work.
Yos hosts the Team with the Queen podcast, speaks on youth work and NDIS practice across NSW, and leads SCA's practice direction. Her lived experience as an immigrant woman shapes how SCA approaches culturally diverse families in Western Sydney. Read more about her work as a behaviour support practitioner on yosmerys.com.
We take referrals from families, support coordinators, DCJ caseworkers, and schools across Western and South West Sydney, with specialist Positive Behaviour Support in the Illawarra.
