For Support Coordinators

A referral partner who acknowledges same-day, reports on time, and does not ghost you.

NDIS provider for support coordinators in Western and South West Sydney. Same-day referral acknowledgement, detailed reporting, one point of contact, and mentors who actually show up week after week.

What we hear

What support coordinators tell us.

  • "You refer to providers who promise the world and then go quiet for weeks"
  • "Reporting is thin, late, or written in a way that will not survive a plan review"
  • "Staff turnover is so high the mentor who built rapport is gone by month three"
  • "Referrals fall through because nobody ever confirmed capacity in the actual postcode"
  • "Providers work around plan management and finance instead of with them"

Direct working relationship with the plan manager on invoicing, budget burn, and stated supports

SCA is an NDIS provider for support coordinators working across Western and South West Sydney with young people aged 12 to 20. We know how much of your day is spent chasing providers who said they would call back, and we have built our operations to be the opposite of that experience.

Our founder Yosmerys Aparicio Cuencas came out of frontline youth work, so we understand what a support coordinator actually needs from a provider: capacity honesty at intake, a mentor who does not ghost the young person, and reports that hold up when the plan review comes around.

What we hear from support coordinators

You send five referrals and hear back from two. You get a mentor allocated who lasts eight weeks then quits. You ask for a report a fortnight before the plan review and get three paragraphs that do not tell you anything you can use. You find out at month six that the provider has been out of budget since month three because nobody was tracking the burn.

You want a provider who acts like a colleague, not a black box. You want to know within a working day whether we can take the referral, within a working week whether we are a fit, and within a working month whether the young person is engaging. And when things are not going well, you want the phone call before the crisis, not after it.

How SCA works with support coordinators

Referrals are acknowledged the same working day, in writing, with a name attached. Within three business days you get a fit assessment: yes we can hold this, here is the proposed mentor or practitioner, here is a start date, here is the estimated budget burn. Or, honestly, no we cannot, and here is why.

Every young person gets one named point of contact at SCA who owns the file across the plan cycle. If that person is on leave, you get a written handover to a briefed second, not a stranger reading the file for the first time on the phone. Session notes land in your inbox within 48 hours of the session. Monthly summary reports go out on a fixed date agreed at intake so you can plan around them.

We work with plan management, not around it. We give the plan manager clean invoicing, expected line items, and a heads-up when a budget looks like it will over or underspend. If a young person’s stated supports are locked to a specific pocket, we track it and tell you before we bill.

What to expect in the first two weeks

Day of referral, written acknowledgement from a real person. Business day three, fit assessment back in your inbox. Week one, joint meeting with you, the family or carer, and where relevant the DCJ caseworker. Week two, first working session and first note in your inbox the following day.

If there is any drop-off in engagement, we call you the same week. No ghost referrals, no silent files.

How to start

Send a referral through /refer/ or email the details to info@supportingcommunitiesaustralia.com. If you want to talk through a young person’s fit before you formalise it, call 0480 800 521 and we will hold ten minutes for you the same working day. We would rather have a five-minute conversation about fit than accept a referral that should have gone somewhere else.
