Why SCA

Behaviour support built for teenagers, not squeezed in.

Most large providers in Western Sydney support every age, from young children to adults. We only work with young people aged 12 to 20. Adolescence brings its own pressures: school, friendships, identity, independence and the move towards adult life. A plan has to fit that.

01

Practitioner-led.

Behaviour support is led by Yos, an NDIS behaviour support practitioner with eight-plus years of frontline practice across disability, out-of-home care and community services in Western Sydney. Meet your practitioner.

02

The young person as author.

Young people help make their own plan through drawing, story and short animations made from their own drawings, alongside a standard function-based assessment. No filming or photos of young people, ever.

03

Restrictive practices with an exit.

Where restrictive practices are in place, the plan names the skills that make them unnecessary and a clear path to reduce them, lodged with the NDIS Commission and authorised through NSW DCJ.

How it works

From first call to a plan that holds.

1

Referral and getting to know the young person

What's being asked, how it's funded, consent, and time to understand the young person: their strengths, what a good day looks like, how they communicate, their health and sensory needs. Rapport comes first.

2

Functional behaviour assessment

Conversations with the young person, family, school and support team, records, observation and simple data, to understand what the behaviour is doing for them.

3

Interim plan, if needed

Within one month when there is an immediate need: safety, changes to the environment, and calm ways for adults to respond.

4

Comprehensive behaviour support plan

Within six months: the full picture, skills to teach with clear steps, proactive and response strategies, and a data plan a real team can keep up with.

5

Coaching and review

We model and coach the plan with family, carers, school and support workers until it holds, and review it at least every 12 months, or sooner if things change.

Safety and connection first, calm responses from adults, and nobody asked to retell what happened.

Young people in out-of-home care

Support coordinators, caseworkers and schools

Clear referrals, plans written for the people who implement them, and coaching until the plan holds across settings.

FAQ

Common questions about PBS in Western Sydney.

Do you provide Positive Behaviour Support for teenagers in Western Sydney?

Yes. Supporting Communities Australia provides NDIS Positive Behaviour Support for young people aged 12 to 20, including teenagers and adolescents, across Western and South West Sydney. It is led by Yosmerys Aparicio Cuencas, an NDIS behaviour support practitioner based in Liverpool.

Which suburbs do you cover?

Liverpool, Fairfield, Campbelltown, Bankstown, Parramatta, Blacktown and the suburbs around them. Support happens where the young person spends their time: at home, at school and in the community.

What ages do you work with?

Young people aged 12 to 20 only. Many large providers support all ages, from young children to adults. Our practice is built around adolescence: school, friendships, independence, and the move towards adult life.

How is behaviour support funded by the NDIS?

It is paid from the Behaviour Support budget in the young person's NDIS plan (called Improved Relationships on older plans). If it is not in the plan yet, we can help you ask for it at plan review.

Who delivers behaviour support at SCA?

Yosmerys Aparicio Cuencas, an NDIS behaviour support practitioner who works only with young people aged 12 to 20. She does the functional behaviour assessment, writes the plan and coaches the family, school and support workers who use it.

Will you work with my teenager's school?

Yes. A plan that only lives at home rarely holds. We meet with schools, translate the plan into language teachers can use, and coach staff so the young person gets the same support across settings.

Do you take DCJ and support coordinator referrals?

Yes. Support coordinators, DCJ caseworkers and out-of-home care agencies can refer directly. We work with foster carers, kinship carers and residential teams as one team around the young person.

How do I refer a young person?

Use the referral form on this site, book a free 15-minute chat, or call 0480 800 521. We will talk through what is happening, check the NDIS plan, and let you know the next step.
